Insurance Processing 

Family HealthServices Minnesota, P.A. (FHSM) is a preferred provider for several major insurance plans in the Twin Cities metropolitan area.  We ask that patients check with their insurance carrier to make sure FHSM is in their care network.  As a courtesy, we are happy to file insurance claims for our patients.

Insurance companies require that we maintain a current copy of your insurance card.  If for any reason you are not able to provide proof of insurance coverage, a refundable deposit will be required for the first visit.  This deposit may be refunded to you upon proof of your insurance coverage or payment of the charges by your insurance company.

You are responsible for your account even if you have insurance; your insurance contract is an agreement between you and your insurance company.

SERVICES RELATED TO AN INJURY THAT INVOLVE ANOTHER PARTY AS THE PAYER:

We file claims to Worker's Comp, motor vehicle and personal injury insurances.  Patients are required to supply the clinic with all billing information to file these types of claims.  If unable to provide this billing information, you may be required to pay at the time of the visit.  Please discuss with the business office the information that will be needed for these types of injury claims.
