You may be wondering...Why do we ask so many questions

We are striving for a future in which all of our patients get equal access to health care.  Hopefully, this will allow everybody that we work with to live longer, better lives. Family Health Services Minnesota will ask everyone the same questions in order to track how well you, as the patient(s), are doing and how well, we as the caregiver, treat all our patients.

Family Health Services Minnesota knows from national reports and from history, not everyone has received good quality care from a caregiver. As such, all of us at Family Health Services Minnesota want to make sure that does not happen at our clinics. For example, we ask everyone about his or her race and ethnic background for the purpose of tracking our patients’ progress and, of course, how well we are doing with everyone who comes to the clinic.

Only by tracking how we treat all our patients and evaluating the consistency of the care we deliver at Family Health Services Minnesota will we learn how to keep improving the service we provide to all of our patients. We want to know you better and ensure that you get the highest quality of care. The following questions allow us to do just that.

During your next visit we will ask you the following commonly asked questions.

  • Where do you live?
  • What is your marital status?
  • What is your religion?
  • Where were you born?
  • When were you born?
  • Do you work? If so, where?
  • What do you do?
  • Whom do you want us to contact in case of an emergency?
  • Do you have insurance?
  • What is your insurance information?
  • What is your race?
  • What is your ethnicity?
Your answers to all these questions help us in our efforts to make the whole health care system work better for everybody.
