Credit Policy
Family HealthServices Minnesota, P.A. physicians are dedicated to providing you with high quality healthcare. In order to maintain that commitment we realize that we must collect our billing in a timely manner. This credit policy information is designed for that purpose.
For private pay patients, we encourage payment at time of service. To encourage this, we offer a 10% discount when paying by cash, check, debit or credit card as long as there is not an outstanding balance on the account. We accept VISA, MASTERCARD, DISCOVER and AMERICAN EXPRESS.
Patients with no insurance are required to pay for all services at the time of their visit.
If your insurance company requires copayment for office or lab visits, it must be paid at the time of service. All other payments are due upon receipt of the first statement.
