Care Manager
| Location of Position: | To be determined. Each Care Manager is assigned one primary clinic within the FHSM system and site assignment will occur following completion of training. | ||

| Hours Requirement: | Full-time, 4 1/2 day work week or 5 day work week. No weekends. |

| Responsibilities: | Coordination of depression care and chronic disease management within the Health Care Home setting. | ||
| Required Qualification: |
CMA, MA or clinical background with a minimum of an Associate Degree or equivalent experience. Experience in depression care strongly preferred. Flexibility and excellent multi-tasking skills a must. |
